Child Abuse / Neglect Reporting form

    Thank you for your concern in helping keep children safe from abuse and neglect. Before submitting an Online Report, review the following bullet points.

    • Call 911 or your local law enforcement agency if you have an emergency or life-threatening situation that must be dealt with immediately.

    • Do not submit an online report for situations needing an urgent response, please call the Intake Hotline at 1-855-323-DCFS (3237).

    • Whether reporting online or via phone, a reporter’s identity is kept confidential.

      • Information is gathered so that if a case is opened for investigation a caseworker can reach out to the reporter to gather any new information and notify of case closure. (Utah Code 80-2-1005 Confidentiality)
        Confidential means that my name and/or personal information will not be provided to the family as the person who made this report. DCFS and its employees will have access to the information I provide, and there may be instances where it is shared with others, such as law enforcement, to help them assess the concerns and contact me if necessary. DCFS will have my contact information to ask follow-up questions to better understand the situation; notify me if my report meets criteria to open a CPS investigation (accepted) or if it does not meet criteria (unaccepted); and notify me when a CPS investigation closes. (Utah Code 80-2-1005).

    • Online reports have a slower turnaround time than phone calls. The hotline number is available 24 hours a day, 7 days a week.

    • All reports of abuse, neglect, or dependency, including false reports, are taken seriously.

      • A false report is when it is more likely than not that the person knew the report was false at the time the report was made. A false report can have legal consequences. (Utah Code 76-8-506 False Report; Utah Code 80-2-610 Immunity from Liability). Confidentiality may not apply if I am knowingly making a false report. (Utah code 80-2-611)

    • DCFS Intake cannot assist with custody matters or case updates. Custody matters, such as parent time, should be addressed with an attorney and/or the court.
